Understanding Financial DisclosureFinancial disclosure is the process by which a patient is made aware of his/her financial responsibility for the kind of treatment selected. It is the policy of Chiropractic Care of Minnesota, Inc. (CCMI) that contracted network providers notify patients and obtain proper patient acknowledgement of member liability prior to delivering non-covered services for which the member will be billed.

Moving or Retiring? Let us Know!When working through an office move, retirement or any other update to your contact information or availability, please be sure to notify CCMI. Most demographic changes can be reported by submitting the Practitioner Information Update Form which is available on ChiroCare Connect. Termination requests due to retirement or relocation outside of the CCMI service area, however, must be faxed no less than 120 days prior to the desired termination date.
