Financial Disclosure Form for Non-Covered Patient Services

The following Financial Disclosure Form and Policy Overview document are designed to help ensure that providers are able to collect all appropriate funds from the patient. Please note that in order to be able to collect any funds for non-covered services from members administered by Fulcrum, a Financial Disclosure Form must be executed by the patient.

Fulcrum makes every effort to help contracted providers gain access to items that address practice, claim and EHR efficiency, and that help educate patients’ about physical medicine. The majority of resources will be found on Fulcrum’s Provider Portal, but some are available publicly here.

Affirmation

At Fulcrum, we affirm that:

• Utilization Management decision making is based only on appropriateness of care, service, and the existence of benefit coverage.
• Fulcrum does not specifically reward practitioners or other individuals for issuing denials of coverage.
• Incentive programs are not utilized to encourage decisions that result in under-utilization.

Clinical Policies

Both portals are secure websites for Fulcrum's network providers.
 

In Fulcrum's Provider Portal, you can access clinical guidelines, patient history (authorizations and claims), plan summaries and fee schedules, administrative forms, and the most recent provider manual. Additionally, the site supports clinic and provider changes within the Demographic Updates tab within the Provider Portal.
